Restoration of name to OOB’s register

1. The process of restoration is only applicable to Optometrists/ Opticians who were previously granted Full registration by the Optometrists and Opticians (OOB), and whose registration was cancelled by the Board under Section 20(3)¹.

2. It does not apply to Optometrists/ Opticians previously granted Provisional, Conditional or Temporary registration.

3. Application for restoration can only be submitted 3 years after the date of cancellation, and not more than once in 12 months.

4. Application for restoration can be made online via the PRS. A restoration fees of $300 is applicable, and can be paid Credit/Debit Card (VISA or MasterCard only).

5. The application should include the following documents/ information upon application:

   1. State the grounds on which the application is made;
      

   2. Accompanied by at least 2 certificates as to good reputation and character of the applicant, signed by a registered person, magistrate or Justice of the Peace; and

   3. A place of practice.

6. As part of consideration, applicant would require to undertake an assessment to re-assess practice-related knowledge. Further details would be provided upon receipt of application.

7. Applicant are advised to contact Board’s secretariat prior submitting the application.

¹ Section 20 (3) Power of Board to cancel registration, etc.

The Board may cancel the registration of a registered person if the Board is satisfied that the registered person —

1. has obtained his registration by a fraudulent or an incorrect statement;

2. has had any of his qualifications by virtue of which he was registered withdrawn or cancelled by the authority through which it was acquired or by which it was awarded;

3. has had his registration in any other country for the practice of optometry or opticianry withdrawn, suspended or cancelled;

4. has ceased to carry on the practice of optometry or opticianry for which he is registered;

5. has failed to comply with any condition to which his registration is subject;

6. has contravened any regulation made under this Act relating to the practice and conduct of optometrists or opticians that applies to him;

7. has been convicted of an offence in Singapore or elsewhere involving fraud or dishonesty;

8. has been convicted of an offence in Singapore or elsewhere implying a defect in character which renders him unfit to remain on the Register of Optometrists or the Register of Opticians;

9. has been guilty of any professional misconduct or negligence;

10. has been guilty of any improper act or conduct in his practice as an optometrist or optician which renders him unfit to remain on the Register of Optometrists or the Register of Opticians;

11. is unable to carry out the practice of optometry or opticianry for which he is registered safely or effectively by reason of any mental or physical disability; or

12. has failed to pay any penalty imposed on him under subsection (4)(b).
