Change of Workplace and/or Supervisor

For Provisional/ Conditional/ Temporary Registration

As stipulated in the supervisory framework, for any change request for workplace or supervisor, you must seek for approval 2 weeks in advance of the change by email to the Board at OOB@spb.gov.sg.

For Change of Workplace & Supervisor

Please note that the granted conditions issued under your registration do not allow you to change your workplace without the approval from the Board.

If there is a change to your workplace during your registration period with the Board:

1. You must download and complete Form P2.

2. Submit to the Board with the subject heading: at OOB@spb.gov.sg;

3. You may wish to check the Eligibility of a Supervisor prior to appoint a new supervisor;

4. Your application will only be processed with complete Form P2 and any supporting documents.

5. You will be notified via email once your request for change of workplace is approved.

While waiting for approval, you are to stop practising opticianry/ optometry at your new proposed workplace until you have received the approval from the Board.

You are encouraged to submit your request two weeks in advance and you will be notified via email.

Please be reminded that the Board has the power to review or cancel any registration if it is satisfied that the registered person has failed to comply with any condition to which his/her registration is subjected to.

For Change of Supervisor only

All provisional/temporary/conditional practitioners must practice under direct supervision of a qualified supervisor. If there is a change to your supervisor during your registration period with the Board:

1. You must download and complete Form P3.

2. Submit to the Board with the subject heading: at OOB@spb.gov.sg;

3. You may wish to check the Eligibility of a Supervisor prior to appoint a new supervisor;

4. Your application will only be processed with complete Form P3 and any supporting documents.

5. You will be notified via email once your request for change of supervisor is approved.

You must stop practising opticianry/optometry immediately upon change of appointment for new supervisor, until the Board has approved the appointment.
