For Primary Supervisor Providing On-Site Supervision
a) For supervisee who obtains registration at any time from 1 October 2022
You shall conduct a regular progress assessment at the end of every six (6) months based on the date of appointment, or earlier whenever there is a change of employment, and submit to the Board for assessment using the supervisor’s report template provided via email at OOB@spb.gov.sg within 2 weeks from the end date of the prescribed assessment period.
b) For supervisee who obtains registration at any time before 1 October 2022
You shall conduct a regular progress assessment at the end of every three (3) months based on the date of appointment, or earlier whenever there is a change of employment or supervisor, and submit to the Board for assessment using the supervisor’s report template provided via email at OOB@spb.gov.sg within 2 weeks from the end date of the prescribed assessment period.
c) Email submission of supervisor’s report
It is also strongly advised for supervisor to follow the following format for the subject of the email and the naming of the file to facilitate secretariat’s processing of the submission :
Subject: Supervisor’s Report for name of supervisee (time period in dd/mm/yyyy) – name of supervisor
For example, Supervisor’s Report for John Smith (01/10/2022 to 30/03/2023) – Jane Doe

For Secondary Supervisor Providing Off-Site Supervision
Secondary supervisors are appointed for supervisees in exceptional work settings to provides guidance on optometric practice, conduct progress assessment and sign-off cases for the supervisee.
a) For supervisee who obtains registration at any time from 1 October 2022
You shall conduct a regular progress assessment at the end of every three (3) months based on the date of appointment, or earlier whenever there is a change of employment, and submit to the Board for assessment using the supervisor’s report template provided via email at OOB@spb.gov.sg within 2 weeks from the end date of the prescribed assessment period.
b) For supervisee who obtains registration at any time before 1 October 2022

Termination of Supervision
If you would like to terminate the supervision of your supervisee, please email the Board at OOB@spb.gov.sg with your request as soon as possible.
Kindly provide the following details:
Subject heading -
Effective date of supervision - Indicate to the Board the date where you started supervising your supervisee.
Date of termination - Indicate to the Board the date where you will cease supervision.
Reason for termination - Explain to the Board in details, the reason of termination of your supervisee.
Kindly be reminded that you must signed off all the cases and complete all reports when he/she was still practising under your supervision.
